Important information regarding your Plum Health + SET SEG Family Healthcare Center membership.

What’s new?

As of Feb. 1, the management of the SET SEG Family Healthcare Center has transitioned to Plum Health, a Michigan-based healthcare provider known for its commitment to personalized, patient-focused healthcare. 

The following provides answers to questions you may have on what this transition means to you and your patient experience.

Current Patients

  • No. Your district will continue to pay for you and your family to access the center.

  • If you have been an active member of the SET SEG clinics in the last 12 months, you do not need to sign up again. Just call your clinic phone number after February 1st, 2024 to schedule an appointment with Plum Health.

    If you are new to the SET SEG clinics, or have not used the clinic in more than 12 months, please sign up through our website after February 1st, 2024.

  • Plum Health is committed to quality patient care and is working through center staffing at each location to balance continuity and elevated service. Any staffing changes will be communicated via email.

  • Yes. All prescriptions filled by a previous provider will remain in place until a refill is required. At that point, please make an appointment to meet with the current physician at your healthcare center location. If a medication consultation is required before refill, a provider will reach out to you to schedule an appointment.

  • During this transition, all hours of operation will remain the same. You will see minor changes to the hours of the Lansing center upon the February 1 transition. See below for clinic address and hours.

  • The best way to make an appointment at the Lansing, Van Buren & Corunna healthcare centers is through the app, Atlas.MD Patients, or on the web through the Atlas.MD Patient Hub. You may also call the same clinic phone number as previously used.

New Patients

  • New members should call their clinic to establish their account and schedule their first appointment.

  • The healthcare center is designed so that you, your spouse, and your dependents will be able to schedule same-day and next-day appointments.

  • Yes. The healthcare center only accepts patients by appointment. This allows for a patient-first approach that eliminates wait times and allows for extended visit times.

  • All locations carry a variety of common generic prescription drugs that can be dispensed during a visit or to refill a prescription.

  • If you are a patient at any healthcare center location and are exhibiting symptoms of COVID-19, then you may receive a rapid COVID test.

  • No, the healthcare center does not provide the COVID-19 vaccine. To find a location near you that provides the vaccine, visit vaccines.gov.

The cost of a visit depends on the plan in which you are enrolled and the reason for your visit:

Non-HSA plan:

  • $0 visits for preventive care (i.e., annual physical)

  • $0 visits for acute care (i.e., to treat an infection, injury, etc.)

HSA plan:

  • $0 visits for preventive care (i.e., annual physical)

  • $20 visits for acute care (i.e., to treat an infection, injury, etc.)

    • This fee is per visit and includes all services utilized during the visit including lab work and prescriptions.

    • $20 visit fee is applied to your deductible if using your HSA card or if receipt from visit is submitted to your insurer.

How much does it cost?

Will my employer have access to my health information?

No. The healthcare center is operated by Plum Health, an experienced healthcare provider that values patient privacy. As with all medical facilities, patient health records and personal information are kept confidential in accordance with strict laws and regulations.
